IOP was 14 for decades. 2023 went up to 21/17 and started on timolol due to family hx. Cataracts noted in 2021. PXE Dx in 2021 and currently 1+/tr. Fuch's Dx Jan 2024 and currently 1+/1+. Epiretinal membrane OS since detached vitreous 2020. Need cataracts out due to horrible night vision. OCT normal OD with borderline nasal thinning OS. Opth considers me at risk for glaucoma based on nerve fiber. IOP now 20/16. Opth no longer does surgery so am seeing a glaucoma/cataract specialist. Regular Opth has ratio as 0.5/0.4. Specialist says doesn't see any Fuch's, PXE in OS only, normal nerve fiber ratio of 0.35/0.35 and wants to do goniotomy at same time as cataract surgery. Has anyone else run into a divergence of diagnosis like this? If I do have Fuch's then surgery could really accelerate it, and with PXE run risk of complications due to weakened zonules etc. so it would be nice if they were on the same page. Surgeon seems to minimize risks, maybe because he has done so many procedures, but it is a little unsettling
